Green & Healthy Homes Initiative Awards Four Pay for Success Projects with Grants and Support

It is my pleasure to announce that the Green & Healthy Homes Initiative (GHHI) has selected four organizations to participate in Pay for Success developmental projects:
- The Community Foundation for Greater Buffalo in Buffalo, New York
- Health Net of West Michigan in Grand Rapids, Michigan
- Le Bonheur Community Health and Well-Being in Memphis, Tennessee
- Partners for a Healthier Community in Springfield, Massachusetts
During the course of each 12-month engagement, GHHI will provide cash subgrants and technical assistance to each awardee and local partners, to develop Pay for Success projects addressing the social determinants of health.
Research shows that the social determinants of health have a significant impact on health outcomes, yet remain disproportionally under-funded in today’s healthcare system. Pay for Success provides one financing solution for many evidence-based, public health interventions, and has the potential to accelerate healthcare delivery transformation nation-wide.
These projects are generously supported by the Corporation for National and Community Service, which—through the Social Innovation Fund—is providing GHHI and our awardees with grant funding to explore these projects.

Green & Healthy Homes Initiative Awards Four Pay for Success Projects with Grants and Support
GHHI will advance four Pay for Success (PFS) projects benefitting low-income families through providing comprehensive services and addressing social determinants of health
Baltimore, MD May 9, 2017—The Green & Healthy Homes Initiative (GHHI), following a national competition, has awarded grants and support to the Community Foundation for Greater Buffalo (Buffalo, NY), Health Net of West Michigan (Grand Rapids, MI), Le Bonheur Community Health and Well-Being (Memphis, TN), and Partners for a Healthier Community (Springfield, MA) to advance their Pay for Success (PFS) projects.
GHHI is providing these grants and services through funding awarded last year from the Corporation for National and Community Service’s (CNCS) Social Innovation Fund (SIF) to support SIF Pay for Success projects that address social determinants of health. These awards will build off of the past work of the SIF projects to assess the feasibility of the PFS financing model and advance high quality, promising projects towards launch.
Each of the PFS projects focuses on providing comprehensive services to asthmatics in low income communities. These services include home-based care and addressing environmental triggers of asthma such as pests and mold. These services will lead to better health and quality of life outcomes for families and reduce emergency department visits and hospitalizations due to asthma, saving health care costs. Prior work has shown that a PFS transaction is feasible, and GHHI will be working with the awardees to pilot the home-based services, work with their state Medicaid programs on any needed policies to advance PFS, and engage partners such as potential investors as the projects advance towards launch.

To truly impact the health of populations, we need to move beyond the walls of the hospital and clinics, and these four projects will establish a mechanism to truly scale services that address social determinants of health.”
